Default I know they have killed people

I know that vault explosions have taken out cars and families. The aftermath they couldn't find the covers! Depends on the force, (amps), circuits in them, where the fault was, too many parameters to count. I know a padmount was found on roof, forced ripped the pad clamps!
every hole is differnt and circumstances are different. How far from the sub. How much load? What's in the vault? How much protection on the circuit or lack of protection? Too variable.

I watched a manhole blow up some years ago, 27.6kv feeder cables from station to riser. The smoke rolled for a good 10 minutes before the cable faulted, then the ball of fire was at least 30 feet high. They didn't find the manhole cover until they pumped the water out and then it was in 3 pieces. I know fellas work in live manholes all the time but I won't , not after seeing that explosion. Survival wasn't an option.

There's a shopping mall near the jersey shore that was built on a land fill/ garbage dump. 19.9 manhole duct system. The holes fill with methane and when there's a primary fault, most of the covers blow off
